A Fan Club is a separate subscription to one verified creator, paid with real money (not Hearts). Creators can offer monthly, 3-month and yearly Fan Club plans, and the plans show their price and any bonus Hearts before you join.
Your Fan Club renews automatically at the same price you paid when you joined, including any discount you got.
Premium includes one Fan Club for free. Any extra Fan Clubs are paid separately and appear as separate charges on your statement.
